Health plans, Medicare Advantage organizations, and Medicaid MCOs are accountable for member outcomes across populations too large for any conventional engagement model to reach consistently. Gap closure. Adherence rates. Preventive care outreach. The work is clear. The capacity to do it at the volume it requires is not.
Manual outreach doesn't scale. Point solutions create fragmentation. And the headcount required to close gaps across a population of any meaningful size is not a sustainable answer to a structural problem.
