This paper presents Q, a comprehensive AI clinical assistant developed by Quadrivia that conducts autonomous, real-time multichannel clinical conversations with patients. Q operates not as a copilot, but as an independent clinical agent under clinician oversight. It is designed to handle the routine, repeatable clinical and administrative tasks that consume clinician capacity, freeing clinicians from administrative burden so they can focus on higher value care.

The inaccessibility and unaffordability of healthcare trace to the same root cause: a structural imbalance between supply and demand that no amount of incremental investment can close. AI offers a viable path to scaling clinical care delivery without scaling headcount proportionally, but only if those AI systems can learn to have safer conversations tomorrow than they had today, automatically, continuously, and at a cost that does not scale linearly with volume.
